Parent's Night Out
Parent's Night Out is a special case which requires an
Emergency Medical Release Form for each child in
addition to a reservation request.
The form must be submitted with a written signature at the
time the child is dropped off.
Blank forms will be available at the time the child is
dropped off; however, a properly filled out form, completed
in advance, will save
everyone time!
