Permission slip & healthcard

You can download a copy of our permission slip to be used for your small group activities if the following guidelines are met:

1. Student Ministries must receive advanced notice (2 weeks) for ALL small group events - notify the appropriate pastor with event time, location, cost, etc.
2. Permission slips must be collected for every student attending with complete information and should be turned in to the Student Ministry office for our records.
